Developmental Disabilities Services  
   

Toomey operates two Intermediate Care Facilities (ICF) for adolescents and young adults with moderate to severe developmental disabilities funded through the Office of Mental Retardation and Developmental Disabilities (OMRDD).  Both homes provide 24-hour supervision, with awake night staff and an extensive clinical team.  The staff to resident ratio is 2:1 during peak hours.  A supervisor is on duty during day and evening hours.  These residences provide a home environment, supporting inclusion into the community, development of daily living skills and opportunities to become as independent as possible.

Home and Community Based Medicaid Waiver Service
Home & Community-Based Medicaid Waiver Services are funded through OMRDD by Medicaid and are also available only to individuals with a primary diagnosis of developmental disabilities.   These services include residential habilitation, both at home and in certified settings, and a 1:1 day rehabilitation program.  These services support daily living skills, job skill development and community integration.

Toomey currently operates one Individualized Service Environment (ISE) on Meadowbrook Drive, Syracuse and an Individual Residential Alternative (IRA) on Peck Avenue, Syracuse.  These residences provide a home environment, which encourages independence while at the same time supports inclusion of individuals into the community and development of daily living skills.  Both homes serve adults.

The Individual Residential Alternative (IRA) in Baldwinsville serves as a dully diagnosed (DD/MH0 female, adolescent population ages 12-18.  Clinical services include a nurse and psychologist.  There are 2 to 3 staff available throughout most of the day.  A supervisor is on duty during day and evening hours.

Medicaid Service Coordination
This service provides advocacy, linkage and referral, service planning and coordination, biannual meetings and contact as needed to ensure clear communication and coordination services for individuals with developmental disabilities.
