Program Description
Provides recreational youth development activities for children ages 5-12. Activities are designed to help children develop better socialization skills and higher self esteem. The children participate in arts and crafts, cooking, dramatic play, athletics, games and field trips. In addition, the staff emphasizes group skills of cooperation, compromise, decision-making and conflict resolution. Summer programming is also available. Contact the individual center for specific information.
Support Services Provided
Tutoring and one-on-one support are available for the children. Parents are provided information on other center programs. Children and families are referred to other agencies on an as needed basis. Staff will help families in this process. Those centers with Respite and Specialized Recreational programs work closely with the Elementary Program to integrate children with special needs into the regular programming.
Eligibility
Children need to be 5-12 years old and reside in the neighborhood served by the center.

| Family Support Project |

Program Description
Provides counseling and support to families. Assists the family to access community resources and makes referrals for therapy, day care and other services as needed.

| Health Van |

Program Description
Provides transportation to low-income families for medical appointments. The van takes women and children enrolled with the Syracuse Community Health Center or its managed care affiliate to medical appointments Monday through Thursdays 8:30 a.m. to 1:00 p.m. Pickup from the home.
Support Services Provided
Information and referral provided by drivers and drivers assistants.
Eligibility
Low-income women and children enrolled with the Syracuse Community Health Center and/or Total Care. City residents are the primary service category. Must be SCHC Total Care patients.
Payment
None Program costs are born by the SCHC and an AmeriCorps grant.
Location and Referral Process
The van runs from 1654 West Onondaga St. to the three offices of SCHC for Total Care. The van will also go to private offices upon SCHC referral. Call 424-1804 for further information.

| Home and Community-Based Medicaid Waiver Services |

Program Description
Provides Medicaid case management to individuals with a primary diagnosis of developmental disabilities to promote independence and integration within the community. The case manager develops, implements and monitors a service plan which includes developing appropriate social behaviors and skills, housing assistance, health management, educational and vocational advocacy.
Support Services Provided
IResidential habilitation, respite and day habilitation as needed.
Eligibility
Individuals diagnosed with a developmental disability and eligible for Medicaid.
Payment
None Program costs are born by the SCHC and an AmeriCorps grant.

| Job Connection |

Program Description
Assists inner-city individuals to locate and maintain gainful employment. Job seekers are screened and efforts are made to match the individual with appropriate employment. Once an individual is hired by an employer, a job coach will work with both the employer and employee to overcome any barriers that might hinder continued employment. Support services provided, mentoring of basic job skills, assistance deemed appropriate and necessary for the employee to remain employed.
Eligibility
Individuals 18 years of age or older living in the City of Syracuse.

Program Description
Provides pregnancy prevention services that help Latina girls discover their potential by learning about educational and career opportunities and meeting with talented local professionals.
Support Services Provided
One on one counseling and home visits are also offered.
Eligibility
13 - 19 year old Latinas. The girls must attend school regularly and live on the west or south side of Syracuse.

Provides assistance to Latinos so that they are able to integrate into the community. Staff advocates for the family at various governmental agencies.

Advocacy, translation, emergency assistance counseling and home visiting.

Provides case management services for individuals with developmental disabilities utilizing a person-centered approach to identify the supports and services which the consumer needs and wants.

Individuals with severe disability as defined by NYS Office of Mental Retardation and Developmental Disabilities and are Medicaid eligible.

Program Description
Provides preparation to preschoolers for the challenges of kindergarten. It emphasizes growth in the areas of social, emotional, cognitive and physical development. Children participate in free play, arts and crafts, reading, dramatic play, exercise/outdoor play and field trips. Snack is provided. At Northside CYO children with disabilities receive special education service through Enable. Those centers that deal with Southeast Asian and Hispanic families particularly emphasize English language acquisition with the children.
Support Services Provided
Transportation provided. Information and referral, support and assistance are provided to the family for other center activities or community services as needed. Both Northside CYO and Hawley Youth Organization provide the service in conjunction with the Syracuse City School's Universal Pre-K Program.
Eligibility
Children ages 3-5 years old. Family resides in the neighborhood where the center is located. There are usually waiting lists for this program. It's best to complete an application early.

Program Description
Provides outreach and support services to Southeast Asian refugee youth and their families. Support includes integrated recreational opportunities, academic support, educational classes and ESL classes.
Support Services Provided
Cultural activities, community service projects, support groups and one-to-one counseling. Bilingual staff (Vietnamese/English and Hmong/English).
Eligibility
Southeast Asian children ages 5 - 18, and their families. Includes Vietnamese, Cambodian, Hmong and Laotian youth. Primarily serve Northside residents.

Program Description
A youth development program offering activities that include athletics, arts and crafts, field trips, information sessions, pool, swimming, foosball, cooking sessions, as well as socialization with peers at the center.

Program Description
Provides tutoring to youth from Bishop Foery Foundation's elementary program by matching the children with volunteers from Syracuse University and other local colleges. It focuses mainly on one-on-one relationships and tracking academic progress. Services are provided Monday through Thursday from 3:00 - 5:00 p.m.

Children ages 6-12 who reside in the Southwest community and attend the
Bishop Foery Foundation elementary program.
